What exactly are claim forms for health insurance? What do they mean?
I never had health insurance before and I need to enroll in one for college. I was about to use the insurance for my wellness visits- examination and vaccinations. However I saw the part where there was a claim form. What are the claim forms? Do I have to pay OUT OF POCKET first and then submit the form with the bill to be reimbursed? I dont have the money to pay out of pocket, only for the insurance premium.
A claim form, is a paper you fill out, that shows what exactly you got done, by whom, so the insurance company can either pay you, or pay your doctor.
Sometimes, you pay out of pocket and hope to get reimbursed. Sometimes, they pay on your behalf. You’ll have to ask THEM which way they do it, if it’s “pay on behalf” or “indemnity”.
